Sponsorship Form - If you are interested in becoming a sponsor please complete the online form. There are several different packages to choose from. You can have your company / organization's logo on our website, and / or field. There is also team sponsorship, please review the form for pricing and details and combinations.

Concession Stand Sign-up Form

Medical Release Form - *Required before the player can take part of any of the practices, games, or events hosted by Otisville Little League.

School Enrollment Form - *Required for Players not residing in the "Otisville" mailing address This form must be presented a Otisville Little League before the 2023 Season starts. It must be signed by the Principal of the Otisville Elementary for proof of enrollment. The Otisville Little League is required by Little League International to maintain the original copy of this form for enrollment, eligibility, and insurance in the Otisville Little League. This form can also be used to show that a sibling is enrolled in Otisville Elementary.

Incident / Injury Tracking Report - To be completed by Coach or Assistant Coach. For injuries that occur while participating in Otisville Little League functions such as games, practices, parades, etc.

Accident Claim form - This form must be completed by parents (if claimant is under 19 years of age) and a league official and forwarded to Little League Headquarters within 20 days after the accident. Please review form before submission.
